Dr Ham was Locum for Dr Davidson.He examined my right foot and in his clinical notes he mentioned left foot.It was difficult to rectify for my claim.

He gives everyone nasal spray and anti-histamines no matter what they come in for. He doesn't write everything down in your chart, so when you come in with a form, it is woefully incomplete or incorrect and you know how hard that is to get fixed!!!!
